The more completely and honestly you answer, the more able I will be to help you and the more smoothly your first consultation session will go! The best method is to read through the lifestyle assessment form and nutri-systems profile (NSP) form without filling them out. Then take a week to complete your daily food log. After the week is over, go back and answer the questions on the forms. The time will have put the questions into your consciousness. Believe it or not, some of us pay very little attention to aches, pains, and the condition of our tissues and basic bodily functions! You do not need to complete any other forms until I advise you to do so.
